Forming committee:

All committee members must hold approved graduate faculty status. This status can be verified at the following link: Graduate Faculty Status Verification

Individuals serving as the research committee member are required to submit an updated CITI certificate. The following link provides access to the CITI training necessary for this role: CITI Training

• Dissertation Committee Form

After successfully defensing prospectus. Proposal Packet Include:

• Dissertation Committee form (PA office has this completed form on file)
• Dissertation Proposal Approval Form
• IRB approval form (IRB will send signed form to you after you send the application)
• Admission to Candidacy Form

Prior to defending dissertation:

• Dissertation Defense Notification Form

After successfully defending dissertation:

• Sample Signature Page Form (this is a sample of what your signature page should look like and the information it should contain, ensure you include the defense date!)
